What are State Plan Supports?

Some State plan supports are offered through Department of Health and Senior Services for the elderly and disabled population in Missouri. This can include services like:

  • Personal care including bathing, dressing, and grooming;
  • Housekeeping, meal preparation, and shopping;
  • Nursing services such as medication set-ups, blood draws, or diabetic nail care;
  • Respite services to relieve a family member or caregiver;
  • Meals provided by the Area Agencies on Aging; and
  • Adult day care programs

 

How/When Would | Be Referred to State Plan Supports?

You would be referred to State Plan Supports, Home & Community Based Supports (HCBS), Department of Health & Senior Services (DHSS) when requesting similar services through Medicaid. This includes services like Self Directed Supports (SDS) or Day Habilitation. At this time, there is no Adult Day Care within 30 miles of our consumers, therefore, this referral does not occur. SDS requires a referral due to the similarity in the services.

State plan supports can link you with Personal Assistant services which can assist someone in household duties, cleaning, cooking, bathing, dressing, shopping, and nursing care. If approved for state plan supports, the service can be utilized independently or exhausted prior to accessing Developmental Disability waiver funded services.
