Why Chart the LifeCourse?
Charting the LifeCourse is a framework created to help individuals and families of all ages and all abilities develop a vision for a good life. The framework is based on the belief that all people have the right to live, love, work, play, and pursue their own goals. It helps individuals and families identify what they need to know and do, find or develop supports, and discover what it takes to live the lives they want to live.
Your Service Coordinator chart the LifeCourse with you and your family at your request. We recommend this for those who are in a transitionary phase of life such as graduating high school, college, moving out, etc. to learn more about the supports or potential for waiver based services available.
