What are the meetings you will have with your Service Coordinator?

Initial Plan Meetings: When you transfer to our agency or receive your initial placement with our agency from the Springfield Regional Office.

One Month Review: After your initial plan meeting, this review will be held one month later to get your family established for quarterly meetings.

Quarterly Reviews: These meetings are held every three months to check in on our consumer’s, their services and lives to ensure they are happy and healthy.

Annual Plan Meetings: This is a once per year meeting to write the ISP (Individual Support Plan)

Amendment Meetings: These meetings will involve the entire planning team to make changes to the ISP-such as, requesting new services or general changes.

 

Why Do We Do These Meetings?

  • We do quarterly meetings to ensure that your family is healthy, happy, and that there is nothing we can offer you.
    • The LinkAbility Consumer Assistance Fund could pay for related needs and care items for your loved one we serve!
  • Annual plan meetings, amendment meetings, & initial plan meetings are to ensure that we can write the ISP as detailed and complete as possible.
    Every year we want to provide updated care, needs, and services information to ensure that everyone understands and can assist in caring for your loved one.
  • Never hesitate to reach out to our office or your Service Coordinator in the event that you require additional assistance, an emergency meeting, or have questions!

What is SWMAP (South West Missouri Autism Project)?

South West Missouri Autism Project

The Southwest Missouri Autism Project (SWMAP) serves individuals in 23 counties throughout southwestern Missouri. Individuals are served through the Joplin and Springfield Regional Offices. In order to be eligible for Autism Project services, individuals must be eligible to receive services from the Division of Developmental Disabilities and have a diagnosis of autism spectrum disorder.

How Do I Sign Up?

Request to meet with your Service Coordinator to discuss potential services, providers, and an amendment to begin SWMAP services! Once approved, your chosen provider(s) will reach out to you to begin your intake process!
