What is a HRST (Health Risk Screening Tool?
The Health Risk Screening Tool (HRST) is a tool used to provide early detection of health risks and destabilization. The HRST tool is a component of the Missouri Division of Developmental Disabilities Health Risk Screening (HRS) Process that has been implemented statewide to support waiver and non-waivered participant health and welfare assurances.
What are the areas we examine during a HRST?
1. Functional Status I. Behaviors Ill. Physiological IV. Safety & Frequency of Services
Who Will Be Your Rater?
A HRST rater is someone who is trained to score and host a HRST
screening
- If you do not receive residential supports, your Service Coordinator will be your rater if you elect to have the screening (this is an optional screening).
- If you do receive residential supports, the Residential RN will be your rater (this is not an optional screening).
Screening Process: - Your rater will ask yes or no questions over the 22 rating areas
- Ensuring that your answers are as accurate as possible allows your rater to arrive at your accurate rating
- Your rater will also need to know about any current or previous medications taken

Why is Health Equity so Important?
As of 2016, an estimated one in four-or 61 million-adults in the United States reported a disability. People with disabilities (such mobility limitations, deafness and blindness, or intellectual disabilities) face many challenges to achieving optimal health and accessing high-quality health care. Data from 2019 shows that compared to people without disabilities, people with disabilities have less access to health care, have more depression and anxiety, engage more often in risky health behaviors such as smoking, and are less physically active.
