What is Service Coordination?

Service Coordination is a free to you service paid through Medicaid funding. It can be Case Management Only or be Waiver Funded and is available to those with active Medicaid/Medicare coverage or South West Missouri Autism Project (SWMAP) services. Your Service Coordinator will assess your need for medical, social, educational and other services, complete or schedule needed assessments, write your ISP (Individualized Support Plan) yearly, conduct quarterly reviews, oversee care and services by providers through service monitorings, and write amendments (changes to the ISP) as needed. Service Coordination is paid through billing completed by the Service Coordinator which is submitted to Medicaid for reimbursement.

What are the Qualifications to Become a Service Coordinator in Missouri?

  • Registered Nurse license or
  • Bachelor’s degree from an accredited college or university

What is LinkAbility Looking for in a Service Coordinator?

  • Meets Missouri requirements for Service Coordination
  • Preferred minimum 1 year experience in the field of disabilities
  • Able to pass an Abuse and Neglect background check
  • Valid driver’s license and car insurance
  • Excellent reading, writing, and comprehension skills
  • A heart to help others and increase their quality of life
