What is Targeted Case Management?

In Missouri, Targeted Case Management (TCM) is a program designed to assist individuals with developmental disabilities in accessing necessary medical, social, and educational services.

What Does This Include?

  • The Missouri Division of Developmental Disability Targeted Case Management provides support coordination, service coordination, and case management services to help individuals gain access to the above services.
  • To qualify for TCM, individuals must be MO HealthNet eligible and reside in a community setting or be transitioning to one.
  • TCM services include assessment, case coordination, case monitoring, and community reintegration.
  • Non-profit agencies, such as LinkAbility, offer TCM services to individuals with intellectual and developmental disabilities.
  • The funding for TCM services is a combination of state and federal dollars, with specific allocation formulas determining funding amounts.
