How Do I or My Loved One Obtain Targeted Case Management Services?
Individuals must have a developmental disability which is attributable to a mental/intellectual or physical impairment which occurred before the age of 22. It must be determined that the disability is likely to continue indefinitely AND that it results in a substantial functional limitation in two or more major life activities.
The Springfield Regional Office determines if someone meets the eligibility criteria. Once the Regional Office determines the person is eligible, they refer the individual to LinkAbility (for individuals located in Taney, Stone, or Ozark county) where a Service Coordinator will be immediately assigned and contact made within 24 hours.
What is the Process for Referral?
1. Obtain a medical diagnosis of a Developmental Disability through a qualified physician with a hard copy for your personal records.
2.Contact the Missouri Division of Developmental Disabilities Springfield Regional Office at 417-895-7400
a.They will contact you when eligibility is determined
3.If you are eligible, you will be assigned a Service Coordinator with LinkAbility and be contacted within 24 hours of assignment to begin Service Coordination
