What is the primary role of a Service Coordinator?

The primary role of the Service Coordinator is to assist you in finding, identifying, researching, and obtaining the services you need to meet your daily needs and goals. Your Service Coordinator can connect you with resources and service providers to help meet your specific needs based on your personal circumstances.

Other Responsibilities & Limitations:
Service Coordinators are also responsible for a variety of other tasks to ensure that services are being delivered properly and that your needs are being met.
Some of the other responsibilities of Service Coordinators include:
• Monitoring and reviewing services to ensure quality and compliance.
• Scheduling state mandated assessments (HRST, MAAS, etc.).
• Scheduling annual plan and quarterly review meetings.
• Advocating at professional meetings (IEP meetings, service meetings, etc.).
• Communicating with other members of your support team.
Service Coordinators are able to assist with a variety of things to ensure that your daily needs are being met. However, there are limitations to the assistance that Service Coordinators can provide. Below are some examples of the limitations to Service Coordination:
• Service Coordinators cannot approve or deny any service requests. All service requests are reviewed and approved by the Department of Mental Health.
• Service Coordinators cannot provide transportation in any capacity.
• Service Coordinators cannot independently complete benefits paperwork on your behalf (Medicaid, Social Security, etc.).
• Service Coordinators cannot provide any kind of legal advice.
• Service Coordinators cannot provide 1:1 direct care support for any individual.

Autism Project

The Southwest Missouri Autism Project (SWMAP) operates on a shared-unit model and serves individuals in 23 counties throughout southwestern Missouri. Individuals are served through the Joplin and Springfield regional offices. In order to be eligible for Autism Project services, individuals must be eligible to receive services from the Division of Developmental Disabilities and have a diagnosis of autism spectrum disorder. The Division requires documentation of a formal diagnosis for verification during the intake process.

How Do I Sign Up?

Request to meet with your Service Coordinator to discuss potential services, providers, and an amendment to begin SWMAP services! Once approved, your chosen provider(s) will reach out to you to begin your intake process!
