What is the primary role of the Consumer & Support Team working with LinkAbility?

The primary role of the consumer is to communicate with the Service Coordinator,
attend meetings, work towards personal goals, and participate in any established
services. Service Coordinators rely on their consumers to provide the information
needed to help find the appropriate resources and services.

Other responsibilities of the consumer (if applicable) include:

  1. Engaging, communicating, and interacting with current and potential service providers.

  2. Communicating any needs, changes, or concerns.

  3. Participating in annual plan development.

  4. Signing paperwork in a timely manner. Providing signatures in a timely manner is vital to ensure that timelines are met when requesting services.

  5. Maintaining benefits and benefits paperwork including Medicaid renewals and Social Security applications.

  6. Utilizing any services you receive. If you are approved for services, but do not utilize them, the services are subject to termination.
